Advancing Medicare 14-Day Reform

We are convening patient advocacy organizations, laboratories, clinicians, researchers, professional societies, health systems, and industry stakeholders across the cancer care landscape to improve timely access to cancer biomarker testing through thoughtful reform of Medicare’s 14-Day Rule.  

The CMS 14-Day Rule is a Medicare payment policy that determines whether certain laboratory tests performed shortly after a hospital stay are billed by the hospital or the laboratory. However, Medicare’s 14-Day Rule can create barriers to timely biomarker testing for patients with cancer. In certain care settings, these challenges may delay access to critical molecular information needed to guide treatment decisions. 

The alliance’s primary objective is to advance policy solutions that reduce unnecessary administrative and billing barriers and improve timely patient access to biomarker testing.

The alliance also seeks to improve understanding of how the rule is implemented in practice. By sharing operational experience, data, and evidence, coalition members will help policymakers and stakeholders better understand the real-world impact and identify opportunities for durable, evidence-based reform. This coalition represents stakeholders across the oncology community and supports improved access to biomarker testing for patients across cancer types.  

Joining this group provides multiple opportunities to participate in advocacy and education efforts, including:

  • Receive updates on policy developments
  • Review and provide input on policy recommendations 
  • Learn about implementation challenges and operational impacts of the Medicare’s 14-Day Rule 
  • Sign organizational letters to Medicare agency and other policymakers 
  • Submit comments during Medicare’s rulemaking  
  • Share evidence and patient experiences that support reform 
  • Participate in regular coalition meetings and discussions 
  • Engage with congressional offices and other decision-makers, as appropriate 
  • Help educate stakeholders about the current rule and the need for reform

Membership in the alliance does not obligate you or your organization to participate in any specific advocacy or educational activity. Members will receive updates about coalition initiatives and may choose whether and how to participate based on their organization's policies and priorities. There is no cost to organizations or individuals interested in joining this alliance.

Advances in precision oncology have made biomarker testing an increasingly important component of cancer care. Ensuring that Medicare payment policies support timely and appropriate access to these tests is essential for helping patients receive the right treatment at the right time.
